Upon the occurrence of a qualifying event, you should notify AIG, CJSC by calling us at 8 (800) 700 8950, (495) 935 8950
or sending the notification to: Claims_AH.Russia@aig.com
and requesting a list of documents required for making a decision on paying the insurance indemnity.
The Policyholder, Insured Person or Beneficiary should, at their own expense, submit fallowing the supporting documents:
request (the request form is enclosed), including their banking details and their personal account number (the account must be denominated in rubles)
copy of the insurance certificate (both sides)
Copy of the passport (the first page and the page with page of residence)
documents proving the occurrence of the insured event
medical records or the physician’s statement
copy of the medical certificate of temporary working incapacity
All the medical records and statements must be on letterheads of the health care outpatient or inpatient facility (or have the stamp affixed), certified by the authorized medical officer (the full name and position must be specified) and sealed.
All the medical records and statements must be submitted either as original documents or as copies certified by the authorized medical officer.
The copy of the medical certificate of temporary working incapacity (if applicable) must be certified by the authorized officer of the Human Resources department.

The insurer has the right to request other documents to the extent they may be required for taking decision on indemnity insurance payment.
